TENANT REQUEST FOR PAYMENT ARRANGEMENTS

  I am placing a one-time request   and promising to make the following payment arrangements.  I understand that I will still owe late fees, as per the lease agreement,  and may still receive late rent notices and/or eviction notices as per office procedure. I also understand that if my circumstances were to change, or I cannot make the agreed upon arrangements, I am to call Debbie Dizmang immediately at (918) 338-0120.

• Payments made will only be accepted online through tenant portal or money order. No checks will be accepted. On time payments will prevent eviction proceedings.
• You will be notified of request approval or denial within 24 business hours of Request Date.
• This will be noted in your Tenant File
